PAYMENT IS NOT REQUIRED to fill in the survey and print it out. The payment is for review by Dr. Watson. You may register without making payment. Once registered you may log in, with enrollment key, then print the results.
You may want to take the printed copy to your local occupational medicine clinic or personal physician, if they provide respirator clearance evaluation. You can also keep a copy for your records.
